The Key to Medication Safety
Welcome back to our third, and final, installment of our deep dive into medication safety. In Part I, Dr. Farah Madhat, PrescribeWellness’ Executive Vice President of Operations, discussed the prevalence of adverse drug events (ADEs). In Part II, we identified the primary risk factors and dangers associated with the emerging public health crisis. Next, in this final installment, we’ll turn our attention to the solution, particularly Tabula Rasa HealthCare’s (TRHC) innovative approach to redefining medication safety through science-backed technology.
As community pharmacists, we believe no one profession is more exposed to the risks and implications of ADEs, nor is any one profession better equipped to provide the necessary medication risk mitigation services so critically needed.
While diet and other health factors affect a person’s response to certain medications, one of the most effective indications of response variability is in fact genetics.1 Pharmacogenomics (PGx), the study of how people respond to drug therapy based upon genetic makeup, provides unparalleled insight into drug therapy optimization. By utilizing PGx, and other pharmacotherapeutic principles, community pharmacists can assess precision dosing and ensure effective care.
Imagine, for a moment, a world where pharmacists are able to administer accurate and effective care through informed decisions, driven by careful pharmacogenomic assessment – ADEs wouldn’t stand a chance! Moreover, personalized care, utilizing solutions and services informed by PGx, can help combat associated healthcare expenditures – it’s worth noting that the United States currently spends more than $500 billion dollars each year combating the negative effects of ADEs!2
As community pharmacy continues to shift toward value-based care, TRHC Chairman and CEO, Calvin H. Knowlton, PhD, saw a unique opportunity for pharmacists to “increase patient safety and revenue by putting science back into their practice.”3 By looking beyond one-to-one drug interactions, Dr. Knowlton and TRHC’s renowned clinical and technological experts, have established the necessary medications safety science for pharmacists and providers to administer the most effective drug therapy possible.
TRHC’s proprietary MedWise™ software solution identifies accumulative multi-drug interactions to help pharmacists and providers better manage patients’ medication regimens. Through science-backed initiatives, risks associated with ADEs can be mitigated by reducing trial-and-error prescribing. MedWise is reinforced by TRHC’s proprietary Medication Risk Mitigation Matrix™ that delivers point-of-prescribing, medication decision support tools, and precision dosing systems that are interoperable with industry-leading electronic health records. Pharmacogenomic results, available within the Medication Risk Mitigation Matrix, can be utilized by pharmacists to evaluate pharmacokinetic interactions in the context of drug-gene relationships, allowing for one, comprehensive and cumulative recommendation to the patient.
Also available through MedWise is TRHC’s MedWise Risk Score™, a quick and easy way to assess which patients are at highest risk for ADEs and require the most immediate medication management attention. The MedWise Risk Score value is calculated using active medication ingredients of a patient’s entire medication profile, including over-the-counter medications.
By utilizing the MedWise Risk Score, a pharmacist is presented with the most comprehensive picture of a patient’s medication regimen. With this picture in view, pharmacists can be better equipped to make meaningful and actionable clinical decisions for the patients they serve.
Though ADEs continue to pose risks for millions of Americans, community pharmacists, through clinical expertise and advanced scientific initiatives such as pharmacogenomics, play an integral role in crisis mitigation.
