You have likely heard us mention this stat in the past, but it bears repeating: Adverse Drug Events (ADEs)—an injury resulting from medical intervention related to a drug—is the 4th leading cause of death in the United States.1 You have also probably heard us mention how many ADEs are preventable, avoided by leveraging the MedWise Risk Score™, a proprietary Tabula Rasa HealthCare (TRHC) technology—available in the PrescribeWellness solution—that utilizes a one-of-a-kind algorithm to cumulatively compare how patients’ medications interact together, all at the same time, discovering potential risks that other solutions often miss.
We have long believed that decision support tools backed by science can enhance the way pharmacists deliver care by enabling safer and more effective medication regimens.
And now there is new scientific data to fully support this view.
The results are in
In conjunction with DARTNet Institute and the Department of Clinical Pharmacy, Skaggs School of Pharmacy and Pharmaceutical Sciences, University of Colorado, TRHC’s Precision Pharmacotherapy Research and Development Institute, recently conducted a study that found a correlation between risk of premature death and higher medication risk scores, affirming that the appropriate use of medications is critical to avoid harm.
Using TRHC’s proprietary MedWise® technology, the researchers analyzed electronic health record (EHR) data of more than 427,000 patients from multiple U.S. healthcare organizations to calculate risk, which revealed that patients with EHR-recorded deaths had significantly higher risk scores than others.2 In other words, this study offers concrete evidence that risk scores are impactful indicators of mortality risk from ADEs, validating the MedWise Risk Score as an effective tool for measuring medication-related risk.
Additionally, this study confirms the important—if not, necessary role of the community pharmacist in advancing and improving medication safety. Empowered with technology to quickly and accurately identify patients who are most at risk for experiencing ADEs, community pharmacists are perfectly positioned to conduct interventions for those with elevated scores, saving lives and improving patient health from where they know best: over the counter.
Medication Safety Review™ – The Next Step
After patients at risk of experiencing ADEs are identified using the MedWise Risk Score, the pharmaceutical intervention can—and should—continue. By recommending patients for a Medication Safety Review, community pharmacists can address concerns and identify personalized, science-backed resolutions. In coordination with patients’ healthcare teams, one-on-one Medication Safety Reviews provide patients with meaningful opportunities to improve medication efficacy, helping to prevent emergency room visits and reduce hospitalizations. Strong relationships, built through enhanced services such as the Medication Safety Review, can keep patients engaged and safe, while also ensuring a leg up on the competition.
