Responsible for millions of avoidable emergency room visits and hospital stays each year, there can be no disputing that adverse drug events (ADEs) pose a national health crisis.
Thankfully, no profession has done more to step up and meet this challenge head-on than community pharmacists. Medication experts and frontline clinicians, pharmacists are not only the ones who are most exposed to the risks and implications of ADEs, but they are also the most well-positioned to identify and resolve them—particularly when equipped with Tabula Rasa Healthcare’s (TRHC) science-backed technology.
Last February, TRHC introduced the MedWise Risk Score™ to the PrescribeWellness software, enabling community pharmacists to identify simultaneous, accumulative, multi-drug interactions and impact medication safety.
To continue guiding community pharmacists with personalized medication decision support through its MedWise® technology, TRHC is proud to introduce the new MedWise Risk Level™ – now available in the Patient Engagement Center at no cost to PrescribeWellness customers.
MedWise Risk Level™
Powered by the same advanced, science-backed medication decision support, the MedWise Risk Level takes medication safety to a whole new level by providing community pharmacists with a fast indication of potential medication risk.
By designating a patient’s medication regimen as minimal, low, intermediate, high, or severe, the MedWise Risk Level streamlines user experience to help community pharmacists identify patients at high risk for ADEs, and who could benefit from a Medication Safety Review™. Pharmacists interested in harnessing the full suite of MedWise medication decision support tools may still access the MedWise Risk Score through accredited MedWise Advisor™ certification.
By using active medication ingredients of a patient’s complete medication list, including over-the-counter medications, supplements, and more, MedWise is able to predict the risk of medication problems and ADEs. In fact, published studies have found that a lower MedWise Risk Score correlated with fewer ADEs, emergency department visits, and hospitalizations as well as lower medical costs and risk of premature death.
Christopher Antypas, PharmD, President and COO of Asti’s South Hills Pharmacy has seen the benefit and value of using MedWise technology day in and day out, noting how the tools allow pharmacists to care for patients in a way they have never been cared for before.
MedWise Risk Level and Your Workflow
Integrating a new tool into your pharmacy workflow can be time-consuming. You may ask yourself: Is it easy to use? Will it fit in seamlessly? Will it help my patients and my practice? The answers are yes, yes, and YES!
With the MedWise Risk Level, your pharmacy staff need only follow these three easy steps:
- IDENTIFY patients at risk for ADEs, directly in the Patient Engagement Center, by viewing the new MedWise Risk Level column, or by looking at the patient profile. Users can also filter by MedWise Risk Level.
- INFORM these patients that a Medication Safety Review is a new service that could help them, or their loved ones, use medications in the safety way possible – users can even leverage complimentary marketing materials and talking points to get the word out!
- CONNECT patients to a certified MedWise Advisor™ pharmacist for a Medication Safety Review.
Medication Safety Review™ – The Next Step
Medication Safety Reviews are specifically designed to help lower the MedWise Risk Level and reduce the risk of ADEs. Conducted in coordination with a patient’s healthcare team, a Medication Safety Review provides meaningful opportunities to improve medication efficacy, which in turn helps to prevent emergency room visits and reduce hospitalizations.
